# What would each approach aim to change?

The combined\-care discussion

Lesson 3: goals and tradeoffs

Compare what each approach aims to change, what it leaves outside its goal, and the risks attached to the proposed operation. Different intended results can explain why proposals differ without deciding which choice is right for you.

Each lesson includes an original fictional explanation and a completed interpretation. Read on screen, in Markdown or with your browser's Print command.

## Put intended goals and tradeoffs together

A plan's goal should describe the intended change, rather than only name a treatment. Compare the stated result and its limits alongside the risks of the proposed operation. If facial appearance is part of the discussion, the extent of change varies between plans. A stated profile goal is not a promised result.

## Fictional explanation: orthodontic\-only goal

This original fictional example concerns an adult whose clinician has already recommended combined orthodontic care and lower\-jaw surgery. The clinician also explains an orthodontic\-only alternative.

The clinician describes that alternative: The aim is to improve tooth positions within your present jaw relationship. This plan does not aim to change the position of your lower jaw. The jaw\-position part of the correction we discussed would therefore remain outside this alternative's goal.

## Fictional explanation: combined\-care goal

The clinician describes the combined proposal: The aim is to address tooth positions and the relationship of the jaws by including a lower\-jaw position change. The intended bite correction includes changing how the upper and lower teeth meet through that additional jaw\-position change. We would discuss any intended facial\-profile change separately, including its limits. These are treatment goals, without a guarantee of a particular functional or appearance result.

## Fictional explanation: surgical tradeoffs

The clinician explains the tradeoffs: Because this proposal includes lower\-jaw surgery, we need to discuss changes in sensation that can persist. The lower\-jaw or chin operation matters to that concern; this statement should not be applied to every upper\-jaw operation. A bite result can also require further treatment, including another operation. We need to explain which risks apply to the particular operation being proposed for you.

## What this comparison tells the adult

The orthodontic\-only explanation offers a tooth\-position goal while expressly leaving the proposed jaw\-position change outside its scope. The combined explanation adds that change to pursue a different bite goal. The additional goal comes with the surgical concerns just described, including persistent sensation change in the relevant operation context and possible further treatment or another operation.

Both explanations can mention the bite without describing the same intended change. That word alone does not tell the adult whether the jaw\-position goal is included. A narrower endpoint does not make a person's preference inferior. A broader intended correction does not establish the better choice. The remaining personal question is how the proposed lower\-jaw operation's risks and limits relate to this adult's goals and priorities.

## Use the difference in your next conversation

Ask the treating team to connect the proposed jaw\-position change to the stated bite goal, then explain what persistent sensation change or further surgery could mean in your plan. Keep your own priorities in that discussion. You can understand what each proposal aims for while continuing to consider its tradeoffs.
